If there are two roles (powers) on the civilian side, does something like a Godfather need to be implemented on the mafia side, for balance? MM's second setup, in Trees, doesn't have this, for instance.

I'm not experienced enough with these setups to answer this with full confidence, but given that the setup of 1 cop, 9 VTs, and 3 VM is about as balanced as it comes, adding in a civilian doctor would require some sort of mafia balance, in my opinion.

I wanted to introduce the godfather into a game, so it seemed like a worthy counterpart.

This is true, but there are some other ideas that impact the game.

Nobody knows that there are is a doctor or a cop except for the real doctor and cop (or if someone doesn't die overnight). So both mafia and civilians can use this uncertainty to play the game differently. I'm not going delve too much into strategy though, because that is the players' prerogative. :P

I think that the civ flip of the don to the role checker makes him a somewhat unreliable cop. Not really an insane cop, but it makes any result questionable. That adds some balance.

Yeah, exactly. Nearly all (or even all, if the GF is never checked) of the cop's checks will actually be correct, but he/she will always wonder whether one of the vanilla civilian checks is actually the GF.
